Intubate a patient? Paramedics v. anesthesiologists?
I know that anesthesiologists use of a laryngoscope in the operating room. Is that as do paramedics in the field?
Physicians (incluing anesthesiologists) and EMT-Ps as the use of a laryngoscope for most intubations. EMT-P are often taught by anesthesiologists and have observation and practice time in the OR. Each level has to meet a minimum standard and then each was gives extra education program focused on techniques that are commonly found / used in their carrers proffesional. Outside of anesthesiologists, ED physicians and nurses, and EMT-Ps, most providers get only the basic method of laryngoscope. The providers listed above who will be using the technique routinely as part of your practice will receive a more in depth education, including alternative methods. For example, an anesthesiologist or a physician will have ED depth training in the use of an area Broncheal for intubation, but an EMT-P will have only a basic familiarization, unless they are doing Critical Care Transport. These are the current methods of teaching EMT-P in my area of intubation – Laryngoscope – Naso-tracheal – Blind: Guiding in the tube sound – Digital: With the fingers in the mouth instead of a scope – Torch Styletto: Using a light wand to see the end of the tube by a glow through Skin – Seldingers: Insert a wire through the skin of the neck and "fishing" in – Biopsy: Related to the Seldinger technique above, but the insertion of a thin, flexible plastic rod through the mouth and sliding the endotracheal tube over it into place. Can also be used to change tubes if necessary. You can also allow the provider to insert a smaller tube, then switch to a larger one where the insertion of the largest first was very difficult – cricothyrotomy: More commonly known as "surgery of the airway. This involves cutting a hole in the neck with a scalpel and pass the endotracheal tube directly into the trachea. These are considered an airway if ultimately no methods or situations ither special. EMT-Ps are also trained in LMA and Combitube airway as an alternate. Also you can use direct laryngoscopy with Magill forcepts Relations Board Affairs to remove blockages of the airway (choking). – David
